FiftyForward Living at Home Services
In addition to enriching the lives of very active adults through our centers and programs, FiftyForward provides a spectrum of services to help those age 55+ who are less active and in need of assistance to remain living independently in their homes.
These individuals struggle with day-to-day obstacles that can be difficult to overcome alone; scarce resources, functional limitations and isolation affect many of our frail older friends and relatives in the community. FiftyForward Living at Home Services help these mature adults to live safely, more comfortably and with a higher quality of life. Caring, professional staff provide information, referral, consultation and access to needed services. We also work with and provide support for caregivers.
Living at Home professionals are just one phone call away and are ready to help you put the "living back in your life.
Below are the services we provide and the contact information. If FiftyForward does not provide the service you or your loved one needs, we will work hard to connect you with other agencies with the appropriate services.
Care Management “ Free in-home assessment, identification of needs, location and connection to services. In-home notary services are also available. In Madison, Donelson and Northeast Nashville, call 860-7180. For other areas of Davidson County, call 743-3418 or 743-3416. In Williamson County, call 376-0102.
Victory Over Crime “ Free care management, legal system advocacy, homemaker services, counseling support, referral to community services, and volunteer services. The victimization must have occurred within one year of referral. To begin the journey toward being a survivor, call 743-3417.
Respite Caregivers and Senior Companions - trained volunteers provide free, weekly in-home companionship so that family caregivers can have short periods of time for rest and renewal, and those who live alone can make a friend. Don't rest until you call 743-3421.
Meals on Wheels “ Weekday, weekend and holiday (Thanksgiving and Christmas) lunches for homebound individuals who cannot safely prepare nutritious meals. Weekday meals are provided on a sliding fee scale based on income, assets and expenses. To "order in, call 463-2264.
Adult Day Services - Social interaction, exercise, engaging activities, nutritional meals, transportation and social services provided in a safe, caring environment for individuals who need structure and support due to cognitive or physical issues. Services are offered Monday through Friday from 7 a.m. to 5 p.m. on a sliding fee scale based on income. Caregivers receive support and encouragement and are assured that their loved one is comfortable and safe during the day. For more information, call 463-2266.
Senior Support Services “ Free in-home counseling services to assist with life transitions, changes and concerns. Professional staff offers a listening ear and suggestions to help you move forward in your life. Community education groups are also available. Call 743-3414 to learn more.
Contracted Services “ services include private pay care management and legal appointment as conservator of property or person. Your ability to care for a loved one may be challenging due to distance, work or family obligations or other issues. Let us help you meet your relative's needs (e.g., select and purchase services, monitor care, be an escort to appointments, make home visits, etc.). For additional details and rates, call 743-3421.
